How do I ensure that medical care providers will know about my Representation Agreement and Advance Directive?
Emergency personnel are trained to look for important documents about your health care wishes by or on your refrigerator. Interior Health offers a free service – they will send you a free “greensleeve” to keep these important documents in, instructing you to keep that folder by or on your refrigerator.
You can also give a copy of these documents to your doctor.
